Completed MDS assessments accurately and within federally mandated timelines.
Coordinate interdisciplinary care plan meetings and contribute to individualized care planning. Maintain up-to-date resident documentation to ensure alignment with care plans and clinical interventions. Monitor and evaluate MDS coding for accuracy to support proper reimbursement and compliance. Educate nursing and support staff on RAI/MDS processes, protocols and regulatory updates. Collaborate with physicians, therapists and care team members to drive positive resident outcomes. Qualifications: Current Texas license as a Licensed Vocational Nurse (LVN) or Register (RN). Knowledge of Medicare/Medicaid regulations. Strong clinical documentation and EMR/charting skills. Excellent organizational, communication and analytical abilities.
