SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
Provide in-home and community-based case management and assessment services to youth with mental and/or behavioral health needs, substance use issues, or juvenile justice involvement, primarily in Passaic and Bergen Counties. Role includes extensive travel, documentation, and service coordination to support youth and families, with a strong emphasis on individualized planning and cultural competence.

1. Provides in-home and community-based outreach services to assigned youth and families.
2. Maintains a caseload and delivers consistent case management support.
3. Completes timely, accurate documentation, including progress notes and care plans.
4. Participates in the development and monitoring of Individualized Service Plans (ISPs).
5. Facilitates skills-focused sessions and support meetings with youth and caregivers.
6. Conducts outreach to re-engage families and youth who miss scheduled appointments.
7. Collaborates with multidisciplinary team members and external service providers.
8. Builds and maintains community relationships to enhance service linkage and referrals.
9. Uses a person-centered approach to help youth and families meet prioritized goals.
10. Attends ongoing training to enhance knowledge of youth mental health, trauma, and service planning.
11. Provides bilingual (Arabic/English) support and culturally responsive services.

CLIENT'S REQUIRED EXPERIENCE & EDUCATION
" Bachelor's degree in Human Services, Social Work, Psychology, or a related mental health field required
" Arabic speaking required
" Minimum of 2 years of experience strongly preferred
" Must have a valid driver's license and clean driving record
" Must provide 2 professional references
